I know it's not a published work, but this is mentioned as "forthcoming" in several books. Does it survive? And if so, will it be published?

Yes, Monolith does survive. Kenneth first started writing it in 1959, and carried on redeveloping it over the years. He often spoke of it in very enthusiastic terms to me, and was about to pass it to me for publication before deciding instead to redevelop it. This redevelopment was, alas, not completed, and Steffi tells me that shortly before his death, Kenneth indicated that he did not wish what existed to be published.

This is a great shame really. I know that in one of his later letters to a friend and associate of mine, that he expressed a weariness with the business of typing it out.
Is the artwork by Steffi that was to be the frontispiece of 'Monolith' (and is featured in the new Starfire edition of Aleister Crowley and the Hidden God) a fairly recent work specifically created for it, or a work already in existence ?

It was painted many years ago, I believe, but may still have been specially created for Monolith. Like the majority of Kenneth's novels, Monolith was first conceived in the 1950s, and reworked over time. Kenneth was about to pass me a "publisher's copy" of the typescript in late 2007, but decided to withdraw it for redevelopment. Steffi's cover design for Monolith survives.
